Family Help Center is seeking a full-time Urgent Access and Educational Success Care Coordinator for an open position in a wraparound service program targeting youth in the Erie County Children’s System of Care. Care Coordinators provide home-based services to assist youth and families with strength-based, individualized service planning and provide linkages, coordination, and monitoring of services to improve youth’s education experience and promote family stability.
Care Coordinators are individuals who are responsible for convening Child and Family Teams from which a plan of care is developed for each child and their families.
Minimum Requirements: Bachelor’s Degree in Social Work or related field is required with a minimum of one year direct service experience working with high-risk, complex need families. Must possess NYS driver’s license, own car, and acceptable driving record.
Preferred knowledge of trauma informed care and impacts of trauma on children and families.
