Urgent Access Care Coordinator

Full time, 40hrs/wk: The Urgent Access Program targets youth enrolled in the Erie County Children’s System of Care, ages 6-17, who are at increased risk for out of home placement. The Urgent Access Care Coordinators assist youth and families with strength-based, individualized service planning and provide linkages, coordination, and monitoring of services to increase the family’s and youth’s skill set in addressing challenges in the home, school, and community environments. Care Coordinators are individuals who are responsible for convening Child and Family Teams from which a wraparound plan of care is developed for each child and their families. The Urgent Access Program provides short term interventions with specialized family support.

Minimum qualifications: Bachelor’s Degree in Social Work or related field is required with a minimum of one year direct service experience working with high-risk, complex need families. Preferred knowledge of trauma informed care and impacts of trauma on children and families.
