Lead Care Coordinator – The Lead Care Coordinator serves an assistant supervisor for the wraparound programs which targets youth enrolled in the Erie County Children’s System of Care. The Lead Care Coordinator will have responsibilities both as a direct service care coordinator and as an administrator. The Lead Care Coordinator provides on-site and in the field mentoring and leadership to care coordinators while modeling quality casework, individualized care and best practices.
The Lead Care Coordinator will carry a small caseload. The Lead Care Coordinator will be responsible for accompanying Care Coordinators in the field for initial case openings, on site evaluation and assessment as well as facilitating Peer Supervision groups. This position will provide support to the Supervisor of Care Coordination in administrative duties as needed. The Lead Care Coordinator will staff and oversee the summer youth program.
Minimum qualifications: Bachelor’s Degree in Social Work or related field and three years of direct service work with at-risk, complex needs families. Preferred experience in care coordination, supervisory or leadership experience, and knowledge of trauma-informed care practices.
