Full time, 40hrs/wk: The Educational Success Program targets youth enrolled in the Erie County Children’s System of Care, 6th -9th grade, who are at an increased risk for detention or Family Court ordered out of home placement. The Educational Success Care Coordinators assist youth and families with strength-based, individualized service planning and provide linkage, coordination, and monitoring of services to improve youth’s education experience and promote family stability. Care Coordinators are individuals who are responsible for convening Child and Family Teams from which a wraparound plan of care is developed for each child and their families.
Minimum qualifications: Bachelor’s Degree in Social Work or related field is required with a minimum of one year direct service experience working with high-risk, complex need families. Preferred knowledge of trauma informed care and impacts of trauma on children and families.
