Care Coordinator – Family Wraparound Program: Full and Part-time: Care Coordinators provide home-based services to assist youth and families with strength-based, individualized service planning and provide linkages, coordination, and monitoring of services to improve youth’s education experience and promote family stability.
Care Coordinators are individuals who are responsible for convening Child and Family Teams from which a wraparound plan of care is developed for each child and their families.
Minimum qualifications: Bachelor’s Degree in Social Work or related field is required with a minimum of one year direct service experience with families. Internship and/or volunteer experience will be considered.
